Parkinson care exposes the problems of fragmented health care systems fast. It demands continuity instead of episodes, teamwork instead of handovers, and proximity instead of distance. In Bergamo, this reality became the starting point for a fundamental shift in how care is organized and how people work together. ATS BERGAMO, the Local Public Health Agency of the Region Lombardy in Italy, chose to act where fragmentation hurts most.
