CIRCE-JA was launched in 2023 to transform the European PHC landscape. Six innovative practices on digital tools and integrated care models are being transferred to 40 sites, with 14 European Member States coming together to strengthen PHC delivery and health system resilience. The overarching goal remains clear: to reinforce PHC and strengthen health systems. PHC serves as a protective umbrella, shielding the social contract that binds the citizens and their institutions.
