This leaflet presents the results of CIRCE-JA, which transferred six best practices in primary health care to 40 pilot sites across 12 EU Member States. It highlights implementation experiences, key achievements and lessons learned, with insights into adapting practices to local contexts, supporting sustainability and scaling up successful approaches. It also introduces the European Primary Health Care Observatory as a resource for continued knowledge sharing and collaboration.
