Short Description

Deliverable 3.2 of the TransCare project presents the second iteration of redesigned transitional care pathways aimed at improving the continuity and quality of care for older adults transitioning from hospital to home. This report outlines the continued development and refinement of patientcentered, digitally supported care models. The second iteration of Deliverable 3.2 starts from initial design done in the first release for consistency and self-containment.
